Absolutely. The AI provides a starting point in the notes box. You can add, remove, or change any part of it, your edits save as you type, and Undo puts back the notes you wrote before formatting.

AI SOAP note drafting is included on every plan, including Brook, the free plan. Each draft uses AI credits from your plan's monthly allowance. If you run out before the month ends, you can add credits or upgrade.

AI-generated SOAP notes are a starting point, not a final clinical record. You must review, edit, and sign off on every note before it becomes part of your official documentation. When you do that, the note reflects your professional judgment. The AI just handled the formatting. Most insurers care about the accuracy and completeness of the note, not how it was formatted. That said, always check with your specific insurer or licensing board if you have concerns.

The more context you give, the better the output. A good summary includes: the client's chief complaint or presenting issue, what you observed or assessed, the techniques you used and areas you worked on, and any plan for follow-up. Even a few sentences covering these points gives the AI enough to generate a solid note.

When you click Format as SOAP with Reed · AI, the summary you typed is sent to Anthropic, the AI provider Riverd uses for SOAP notes, through Vercel's AI Gateway, so it can be turned into a SOAP note. Our requests are set so that Anthropic does not keep them after it answers, and they are not used to train AI. Riverd does not keep a copy of your summary or the AI's reply in its logs; we record only the time, size, and cost of the request. The formatted note replaces your text in the notes box and is saved on the session, which is private to your practice, and Undo puts your own words back. Even so, leave client names and other identifying details out of your summary.
