I find that taking these few minutes to get to know each other are key to the beginning of a wonderful practitioner-client relationship. This helps us get to know each other and see if it is a good fit.  Why a Client Intake Form? I want/need to know your medical history, habits, and other pertinent information to make sure I can keep you safe and comfortable throughout your session(s).  About Me I have seen and experienced firsthand many of the stresses that come with balancing a busy career and family. I know the stress that this can place on the body, mind, and spirit.
